This documentation to be presented is for Foreign Citizens who do not have legal residence in Spain (DASE).

What requirements do I have to meet?

  1. Have exceeded the period of stay, i.e. more than 90 days have passed since their arrival in Spain.
  2. Demonstrate a minimum of 3 months of census registration in Spain.
  3. Not having a valid residence visa.
  4. Not being required to provide proof of compulsory medical coverage by any other means, in accordance with European Union law, bilateral agreements and other applicable regulations.
  5. Not being able to transfer the right to health cover from their country of origin or provenance (citizens of the European Union, Chile and Andorra).
  6. No third party obliged to pay.

What documents do I have to submit?

  • Application: Available in two formats: digital form and printable and fillable application form.
  • One of the following identification documents:
    1. Passport (clear copy of all pages completed. In face-to-face procedures, the original must also be presented).
    2. Identity card from home country (only for EU citizens, if no passport has been provided).
    3. For children under 14 years of age without a passport: birth certificate or family book -original and legible copy of the book-.
  • Volante de empadronamiento:
    1. Proof of a stay in Spain of more than 3 months (This certificate must be recent: issued less than 3 months ago).
    2. Registration in any municipality in Spain may be taken into account.
    3. In the case of manifest impossibility of registration or if there are delays in its issuance, a report issued by a social worker of the Public Administration (City Council, Public Social Services Units and Centres, Primary Care Centres, etc.) can be presented, indicating the reason for the absence or delay in the issuance of the Padrón and the date of arrival in Spain.
  • Certificate of non-export of entitlement to medical care issued by the country of origin (only in case of coming from or being affiliated to the social security system of a country of the European Union/European Economic Area/Switzerland, United Kingdom, as well as Andorra and Chile).
  • If the application is submitted by a representative, the representative must provide documentation proving representation of the person concerned (family record book, birth certificate, incapacity certificate or power of attorney) or an authorisation signed by the person making the application. The representative must present identification by means of an identity document: DNI or TIE.

How do I apply?

Face-to-face processing at the health centre

At your local health centre, they will provide you with information on the procedures to follow and give you the information leaflet and the form to fill in.

If you meet the requirements, you will be assigned an appointment at the Processing Unit that corresponds to your registration address, where you must submit all the requested documents.

Electronic and face-to-face processing

The process of this procedure can be carried out electronically or in person. If you opt for electronic filing, you will need to have one of the recognised electronic signature systems.

Do I need a health card?

Under the new regulation, the right to public health care is separated from social security contributions. This means that it is not necessary to have a health card or to be registered in a specific place to access this service.

Processing of the application

Upon submitting the application and the required documentation, you will be provided with a provisional Accreditation Document, which will allow you to access the Health Services. This document is valid for a maximum of 3 months.

Once the application has been assessed, if you do not receive an unfavourable notification within 3 months, it means that the application has been approved and the final document will be sent to you or given to you at the primary care centre.

If the application is rejected, you will be notified within a maximum of 3 months. This will result in the cancellation of the provisional accreditation document and the invoicing of the assistance provided to you during the processing period.
